Table of contents updates

1. Table of contents disappears when page width is too small (so it doesn't appear on mobile). We could make it so on mobile, the table of contents appears as a list above the normal page content?

2. Table of contents is currently on the following pages:

We will also want to eventually add it to the other program/ accepted work pages when they are created.

3. There is a bug on the page https://assets22.sigaccess.org/code-of-conduct.html. The heading 'What Should I Do as a Person in Authority?' link doesn't work.
