Mask ODK forked app as a POST app

The ODK app has a lot of confusing features that we don't need. For instance, we don't need to be able to download a bunch of different forms... we only need one, and we'd like the operator to only have to enter their name and plant once, and the form should be put into the sending queue immediately after it is filled out... not wait to press another "send forms" button. Start this by creating a full wireframe of the app.
