Dedicated editor view

We need to create an edit view. From @MeghanUX:

Some Notes:

• the content is presented TWICE - once in a "view only" box, and again, below in EDITABLE boxes (both boxes can be edited)
• In the grey area near the top - indicate any info we can gather about the "last known edit / change"
• add a NEW box - "Notes" - free-form text entry. "Date/time" is added later, automatically and will display in the "Notes" area near the top after submit.
• THREE buttons - submit, save, cancel (save not indicated in the image)
• please pull over ALL the info from the Translation request and display on this page in some reasonable way
• please change the labels on the text content areas - not "source" and "target" but instead just a description - like "Original content: ENGLISH" and "New Content: English"
• please, use just "SPANISH" and not "Spanish (Puerto Rico)"
