Alpha data collection

Per a conversation with @serundeputy I'm going to rough up an alpha data collection form so they can start collecting data. This will probably be the simplest of forms that pushes data to CiviCRM (because I can do this very quickly) and we'll need to determine how/when to import it as entities later. Perhaps not ideal but needed sooner than later.

Otherwise they'll resort to using a Google Spreadsheet or something like that...

I set up a new branch called alpha and put that live for this early data collection phase.
