Tested localy in singleplayer an multiplayer, can't produce the bug. Any more informations would be nice e.g. what other mods, MP/SP, hud version, hunger version, etc

Only explanation I know right now would be lost packets (caused by what ever?)

You have at least 5-6 mods that can cause huge lags. Could you test it without these mods: "carts, compass, item_drop, mobs, throwing, wieldview" ?
