The state machine doesn't expire the upgrades in executing state, with the expectation that post checks will transition the status.
If Blazar daemon is killed when the upgrade is in EXECUTING state, after restart the upgrade won't expire.
There should be reaping logic to clean such state, but only when the upgrade is actually finished.
