[topicmappr] don't default to map-per-topic when --out-file is used #280

Topicmappr writes a per-topic output map by default, even if --out-file is used (a combined map of all output changes). --out-file should disable this with an additional flag (e.g. --individual-maps=true) if per-topic maps are desired.
