  1. Change User name to: Your name
  2. Add email field: Optional field
    3. Denise to provide the caption for email field

Removed 3. as we were going to place the Privacy Notice as a caption but it's in the text above the form so no need.

  • Change 'Submit Problem Report' to 'Submit Feedback Report'
