Launch scenario's should have a formal state

The extension to FHIRcast that describe its integration with SMART (https://fhircast.org/launch-scenarios/), does not have a formal status and version. This is an important element of the overall specification and should have both.

I'd thought that all the SMART stuff was in the spec, in addition to the informative launch scenarios page. For example, a hub.url SMART launch parameter is described here: https://fhircast.org/specification/STU2/#session-discovery

Are you sure that we're missing stuff? If so, what?
