The p-mosfet has source and drain switched in the circuit diagram, the gerbers, and the production PCB.
There are two good fixes:
One is to flip the mosfet on its back, rotate it around the gate pin (which is correct) and maybe bend the pins down or just blob solder on to connect it.
This doesn't scale well to 550 badges, so we ordered a Schottky diode array with the diodes pointing in the correct direction (common cathode? -- check me!) and did the replacement.
Anyway, this needs to be fixed in the BOM/circuit diagram and the PCB.
