Support for single page apps

Currently the cookie monster will naively rewrite all links to route a redirect through DigiTrust. It's possible that this would negatively affect UX for SPAs, for example

• user visits domainx.com/foo.html
• we show the windowshade, rewrite links, etc
• user clicks on a link with a javascript action

How should we handle?

• do not rewrite such links (conservative)
• do it anyway, UX be damned
