We would like to have chapters submit a form to confirm they are an active chapter. We would like to keep it basic and include the following questions.

  1. Is your chapter currently active? (Meeting on a regular basis and conducting chapter activities)
  2. What are a few examples of what your chapter does?
  3. Do you have any feedback or concerns for the IYNA?
