Magento uses a minimal layout (short footer, short header) for the checkout page (page layout "checkout").
With your provided XML the page layout gets changed to 1-column, having the search bar etc. in the checkout.

#6 fixes this.

I was searching for hours until I come to the same conclusion.
Plz merge this fix asap and do a new release. Same fpr CashonDelivery Extension
