Request: Re-design receipt upload form.

There are specific categories of things we are accepting receipts for FinAid, and we'd like this to be clear on the form, to reduce the number of unnecessary receipts that are uploaded. The categories are:

• Local transportation
• Flight
• Lodging (for non-PyCon registered hotels)

We would like to accept only PDFs and JPGs.

1. We should also mention that receipts should have the name of the recipient, or give an explanation
2. Conference travel dates → let recipients know on form that we will double-check against their flight receipts/hotel receipts
