[Admin Panel] Need Edit User Achievement Page

Talk to Ben about the stuff there. Things I know we need:

• delete photo (if inappropriate)
• edit story
• revoke achievement

What is the "Choose File" for? Is it supposed to add/replace the image associated with the achievement? It's generating a server error right now. (see attached image).

Not sure what "comments disabled" is for, or if we're actually using it in this version...

The page is now formatted like the rest of the admin pages. Still need to add/verify functionality.
