Yelp-style Partner Modal

We would like to turn the partner request for help form into a modal. It would show a list of our partner organizations (all selected by default) and the same name and email address that we have currently.

The form would let the user decide which agencies have their permission to reach out and contact them about their concerns as a tenant.

Actionable items:

1. Make the form collapsible, see https://material-ui.com/components/modal/
2. Open it up with a modal with a clear Call to Action button with the text "I'm Looking for Help!"
3. Create a multi-select drop-down of all the housing advocacy organizations that allow users to select whether they want to share information they enter about their tenancy & subscribing to newsletter
