Feature Description
Reformat any Add forms so that they take in fields matching whats stored in the tables.

Requirements

  1. Check the add Volunteer form and make sure it matches with the volunteer table. If not add in the missing fields into the form.
  2. Check the add Rides Form and make sure it matches with the rides table. If not add in the missing fields into the form.
