Problem to render buttons an input type text

Hi, I've worked with your tool, (the best pdf maker that I found).
I have to create a pdf with buttons and input type text but when I get the pdf document. These elements disappear, how can I add these elements to the pdf?

I've checked today and I can't import radio buttons and selects, how can I add to my pdf these elements?

PDF Form fields is under not done yet. See http://cloudformatter.com/CSS2Pdf, what’s in development. We have had no time (or money) to do so currently. Ok can I help you with something? Because I tried to change some aspects to adapt with the form elements, but I couldn't. Other mistake is when you have an image inside of a div and then you create your pdf, It's visible. So I'd like to contribute, tell me how.
