Improve the live preview so it supports the opinionated include directive module structure implemented in Antora. See also asciidoctor/atom-asciidoc-preview#300
and asciidoctor/asciidoctor-browser-extension#299
This enhancement would increase adoption of AsciidocFX with Antora users.
