Describe the task
The Form IO icons styles are overridden to display properly since the Form IO upgrade in story #4132. There seems to be an issue between bi vs fa icons as shown below, and custom CSS is required to display bi icons.

Acceptance Criteria

  • Investigate the issue and display the icons directly from Form IO.
