In running text, the interpreter will need to see if there is a prefix or suffix and include those if present:

Some text and a [pdf-link: link destination], and more text.

should translate to:

.PDF_LINK SUFFIX , NAME DESTINATION

And

Some text and a ([pdf-link: link destination]) and more text.

to:

.PDF_LINK PREFIX ( SUFFIX ) NAME DESTINATION
