I've included this in the G Form when I removed the extension from my browser, but thought it's worth mentioning here.
I host plex on a home server, so I'm never accessing plex from localhost or 127.0.0.1. When I opened plex and tried to grant permission to access my instance of plex, the 'yes' button didn't work. The 'no' button does work, to deny access to the plex instance however.
A personal solution to it was downloading the git zip (thanks for making it open source btw) and adding the url of my plex instance to the manifest json.

I had the same issue yesterday. However today I found that the permissions prompt actually appeared on a different browser window than the one I was interacting with Plex on. This window was minimized, so I didn't see the prompt until today.
Note: I use a Mac, so there was no indication that the minimized window was asking for attention. Windows might behave differently.
I still can't manually add or remove URLs from the "Manage Extension" > "Site Access" page, but at least this works for now
