Add a table of content

It would be useful to have a table of content with links to the different sections.

I haven't made ToC using GitHub, any suggestion to do this in best way?

Yeah, i know the basics of Markdown.
I'm wondering if GitHub has kind of tool that will create ToC with one or few placeholders... like {table}

There is at the moment no placeholder like {table} : see isaacs/github#215
But in the comment of this issue there is some solutions with external tools.
