expand multi-measure capability

new feature expansion to multi-measure evaluate request:

1. individual measureReport mode, that allows for individual MeasureReport creation for each subject identified via the subject parameter
2. consolidated practitioner parameter capability into subject parameter, for a single population parameter. This allows for simplified explanation and use of api
3. canonicalUrl Measure parameter that allows for both measureId and measureUrl to be used to identify the Measure to use.
4. Research implementation of identifier parameter, implement if available

This allows for reduced redundancy across api calls that will generate MeasureReports as well as additional coverage of resource identification for measures.
