[wtwg review] Version of HLS should be indicated in Annex D

Annex D assumes a methodology of ad insertion (via discontinuity markers in media playlists) that applies to the current version of HLS (v8). To protect the guidelines against a potential future change in how advertising is supported in HLS, the guidelines should explicitly reference the HLS version to which these guidelines apply.
