Form changes

This release is a combination of fixes plus slight changes to prevent potential problems.

Includes:

• Fix for title/description not allow validation due to being perceived empty
• By default the form is disabled for all DOI's created outside the form itself, this is to prevent accidental metadata changes.
• DataCite staff can enable form editing manually in specific usecases.
