API номинатима — отстой.

Варианты:

использовать дополнительный инстанс номинатима
пытаться кешировать bbox'ы результатов (или даже геометрию результата) (ненадёжно)
написать свой геокодер регинов (не, ну а почему бы и нет, в StreetComplete свой для стран написали)
....
