Patient History: Add clinical notes to patient history setting

Description
Currently, "Clinical Note" is not being recorded in the Patient History. This feature request is to ensure that any Clinical Note added to a patient's record is also reflected in their history for better tracking and continuity of care.

Expected Behavior
When a Clinical Note is created or updated, it should be added to the Patient History.
The entry should include relevant details such as the date, doctor, and summary of the note.

Steps to Reproduce
Create a new Clinical Note for a patient.
Check the Patient History section.
The Clinical Note is not visible in history.
