Unclear if app is connected to wallet. Put wallet connection CTA / Status in top right corner of app

The sample app presently places the "Connect with wallet" CTA in the centre of the app, and when connected, the only visual indicator to the user that a wallet is connected is by the user inferring based on the presence of the ":x: Disconnect wallet" CTA.

Let's add the wallet connection status in the top right hand corner of the app. Examples of this appear below. The youves example offers a drop down to disconnect the wallet, and to open the wallet address in a blockchain explorer.
