When I have a item in my hand with the attribute "/ie attribute add max_health" in offhand
We must remove it from the hand and put it back to reactivate the max health attribute.

might be a clientside issue, can you do /itemeditinfo , and paste here the screenshot?, also which version of client are you using (1.21.4 / 1.21.2 / 120.4......) ?
