Add developer / contributing docs

One thing missing from this project is a doc that goes over how you can go about adding a feature, fixing a bug, etc. and what's more adequately testing it (i.e., against an actual instance of hexpm).

Some of this documentation already exists in various places, so it should just be job of collecting and polishing.
