Deprecate support for Django <2.2 #43

There are some updates I'd like to make e.g. #42, which would require dropping support for unsupported versions of Django.

@frankwiles would you be okay with making a major version update that breaks support for legacy Django? I'll help with PRs.

Hey @benmurden sure that's fine with me! I haven't had much time to give django-app-metrics much love lately so the help is appreciated!

Made PR #44 to address this and perform many other updates to get us to Django 2.2.
