Patient, variant and disease should be unique, so ensure that it doesn't exist before submitting form.

May be worth searching during the form steps that we don't already have this combination - not sure if warning that they will override existing variants is a good call - or after we know the patient and disease (AKA reason for test) we should automatically prefill the variant form with the existing information
