On long indexes it is more practical to have an alphabetically ordered index rather than a positional one.

That's more of an index of topics that you might find at the end of a document in an appendix than a table of contents. The alphabetical index of topics destroys the structure of the document. So it seems like it's a different animal from a TOC.
