[ProblemList] !!! Be approach on how to exchange problem lists?

For the innovation project where a problem list consolidator is build, we need to define how to exchange problem lists (e.g. between GP and hospital).

This has been shortly discussed in some WG but I would like a general 'approach' so we are all on the same line (and we can start working on our consolidator).

The idea/proposal
Each item within a Problem or Summary List could be represented using its appropriate resource (Condition, Procedure, Observation, FamilyMemberHistory, AllergyIntolerance, etc). The resulting set of resources are then wrapped up within a List resource where List.code = problems. Since List.code is already =problems, we don't need to use the Condition.category 'problem-list-item'.

Ideas? Suggestions? Remarks?
Can we discuss this when the WG start again?

Remark: Is it useful to add Allergies to the problem list since these will be added vaults? Is this a different list? An allergylist?
Not all packages have allergies added to their problem list.
