In stead of R5, I think you mean R6, https://hl7.org/fhir/6.0.0-ballot2/relatedperson.html . However, we cannot preadopt these changes yet, until R6 is released.

Yes, I see it's in R6 and not R5. Why couldn't we pre-adopt to R6? wouldn't that be like a regular Be-extension with the knowledge that it might be part of the international specifications one day.

I don't want to make it more complex it's merely a matter of trying to understand.

We cannot preadopt it in the technical sense of the word (using the normal extension mechanism) until it is officially released. We can however, build something ourselves, not using the official mechanism, but that equals (on a technical level) a custom Belgian extension, without the possible transition benefits of the official mechanism. Decision is up to the group.
