Pivot total column should be configurable by pivot column

For now displaying the total column of a pivot is a global table's configuration property. It would be necessary to have it defined by level of pivot.

For instance let's say we have 2 levels of pivots: country and canDesign we may want to configure the country to have its total column at "start" position and no total column for canDesign.
