DITA topics must have a title, but there should be error recovery to handle Markdown files that don't have a top level heading.

While DITA topic requires a <title> element, the DTD doesn't require that there's anything inside the <title> element.

<topic id="topic_hrh_j2z_3bc">
    <title></title>
</topic>

is a valid DITA topic. This feature could be implemented by just leaving the <title> empty and throw a warning about empty title.
