To make it more convenient to support other file names, this could also be supported:
file:///.../README.html?file=./foo.md
If present, the file value form the query string would be passed called with GET instead of README.html.
Icing: a little CLI tool could be used to set up the initial HTML request:
preview-markdown --reload 5 ./foo.md
Then there would be zero copy/paste/modifying of files to get going.
Oh, there are already several of these types of things, I was just searching for them the wrong way:

You have to install 'npm' first to install it, but that's generally useful anyway.
