Describe the bug

For lecture attachments, they are added twice when clicking on "save attachment".

To Reproduce

  1. As an instructor, go to course management.
  2. Go to Lectures > Attachments
  3. Click on "Add Attachment" and choose a file.
  4. Click on "Save attachment" and notice the two added files.

Expected behavior

Only one attachment is created.
