Resolve all npm audit errors

Run through all packages and resolve any errors reported by npm audit. Packages with issues should be fixed, updated, published, and consumed by its dependents.

Resolution will usually only involve updating dependencies. Create additional work items for anything involving more than a simple dependency update (such as an alternative).
