Enabling CSS classes does not seem to work in filter.php.

I must disagree. I just tested this on recent Moodle 2.6 and it works as expected. If I look at the HTML produced by the filter, there are classes like br0, re0, st0 surrounding the parts of the code. It's up to the theme to define the actual look and feel of such classes.
