Introduce Azimutt, Mermaid and enhance PlantUML for ERDs

See Azimutt JSON import format and generate Azimutt files to load into azimutt.app. Their ERDs are visually appealing and easier to browse / understand than most. Azimutt also performs SQL linting so we can integrate that if appropriate.

We can also consider emitting MermaidJS and other ERD formats as well as enhancing PlantUML ERDs.
