Util to create normalized clinical data

This is related to nf-osi/nf-metadata-dictionary#334. Once the target normalized clinical data is defined, provide a util to create data in the shape of this target table given denormalized data, possibly from several tables/sources. Probably will need to handle things like:

• Missing specimenIDs.
• Coalescing data, e.g. in some tables there will be more clinical data than others
• Resolving inconsistent data, e.g. same individualID and specimenID, but different sex or age, etc.
