Take the following code which uses recursive imports in the limited form they are currently supported:
a.nim
const Constant = 0 import b
b.nim
import a proc test*() = discard echo Constant
In a.nim nothing from b.nim is suggested, while in b.nim everything is suggested twice.

this seems to be not a nimsuggest issue afterall. Most likely this fixed it: pragmagic/vscode-nim#180
