404 Error For Consultations After a Patient is Discharged From Care #2597

Describe the bug
When a patient is discharged from care, then the user is being redirected to not-found page instead of previous list of patients page

To Reproduce
Steps to reproduce the behavior:

Go to '/facilty'
Click on view patients
Select any patient with consulation
Click on manage patient and select discharge from care
See error

Expected behavior
User should be able to retrieve the consultation page even after being discharged

I'm not able to reproduce this issue, following the same steps.

I just checked it again and looks like it has been fixed :)
