Is your feature request related to a problem? Please describe.
Labels of 'Notify' and 'View Facility' buttons on /facility page are hidden on less that 768px breakpoint.
Users may not understand the button's purpose.

Describe the solution you'd like
Adding a tooltip ensures that users understands the button's purpose
