Additionally, I think updating the Making requests and Unhappy about the response you got? pages to include the following might be an idea:
Internal Review is not available if an agency does not make a decision on time. Perhaps applicants can kindly ask the agency to make a section 15AC to the OAIC in order to reinstate their right to an internal review.
Or a page dedicated to no response, like seen here
Guidance on what to do if an agency asks an applicant for an extension of time. IMO in most cases it would be preferable to extend the time (primarily to retain review rights) however in some circumstances it could be beneficial to allow the agency to run out of time (charge can no longer be imposed, even after timeframe is reinstated by the OAIC).
