Removal of stewardship patients from the list stewardship list

ok, update.
The issue is that this plays a bit funny with episodes. Removing patients from lists, ends an episode, and if they get added back on, then a new episode is created with old clinical advice not visible.

In some ways this is the same problem that micro haem used to have. We could for a first run, just tell Surjo about this issue and that the easy solution is to click "Micro-haem" view. I think this is a good solution, it works (as in data collection is good and valid, etc...) and allows some real world suggestions on what may work better.
