Consider recognizing deprecated exception handling instructions

Currently if I decode a module with the old exception handling instructions I get strange decoding errors and it's hard to see what's going wrong.

It's hard to even find the spec for the old proposal now, but it might be helpful if we at least recognized the encodings and print a helpful message saying that the module uses deprecated exception handling instructions that we don't support.
